Inspection Trouble For Columbus Schools

Of the district's 128 schools, almost 50 did not pass inspections due to various problems, Seavert reported.  Fire inspectors found door hardware that was sometimes broken, blocked or locked.  One of the most common violations was fire extinguishers that expired.  In addition to schools missing monthly fire drills, 10 Investigates' records showed some schools did not post evacuation or exit plans which could cause confusion in case children file out fast in an emergency.

Denis P. Laroque, West Warwick's fire marshal, said in an interview with investigators five days after the February 2003 fire that he had concentrated his inspection the prior November on the club's failure to fix a previously cited problem with an exit door, improperly mounted fire extinguishers, and an open gasoline can in the basement.

Dubai Makes Extinguishers Mandatory in Cars

The Dubai Civil Defence and the Roads and Transport Authority are studying a proposal to make fire extinguishers mandatory in vehicles, reported Gulf News. There has been a significant increase in the number of car fires over the past five years and 77 have been recorded in the first five months of 2006. The ruling would mean that cars not carrying extinguishers would not be registered by the traffic department.

The report also criticized the district for an "apparent lax attitude towards fire inspections and Fire Safety, most especially evident at the High School." St. Helena Fire Chief Kevin Twohey conducts yearly fire inspections at each school. The schools are given advance knowledge of the date of the inspection, but Twohey found violations at all four schools.

Biker bar busted for fire code violations

The bar - whose name and attitude seems to resemble Hogs N Heifers, the iconic biker bar in Manhattan's Meatpacking District - also was cited for 17 fire code citations, which resulted in it being closed.

Among them was a faulty fire alarm system, missing fire extinguishers and a 315-person crowd inside the bar Tuesday that far exceeded the legal occupancy of 50 people, police said.

After the Station Night Club,  you would think bar owners with pay more attention. 

Code Violations for Recycler

After fire,  OSHA violations come to light.

CBCL was one of the businesses damaged last month when the former mill building caught on fire. Acidic smoke from the fire caused the mandatory evacuation of half of Great Falls' 2,200 residents for several days.

The smoke contained hydrochloric acid that resulted from the burning of polyvinyl chloride in CBCL. Water used to extinguish the fire mixed with the plastic chips to form the hydrochloric acid, according to the state Department of Health and Environmental Control.

Arizona:

Quentin Elementary School, 11050 W. Whyman Ave., suffered a “considerable amount of damage” during the break-in, which occurred in the early morning hours of June 10, said Lt. Dale Nannenga of the Avondale Police Department.
